Following feedback from the Western Health Suicide and Self Harm working group, a minor change has been made to the Nursing Initial Patient Assessment Powerform in the EMR.

The Patient Health Questionnaire – 2 (PHQ-2) Chapter will now be mandatory in line with other Chapters containing important patient information (e.g. End of Life/Advance Care Planning). The responses to the Questionnaire will inform Mental Health care considerations to support the provision of Best Care. The PHQ-2 Chapter will need to be completed to enable the form to be signed off.

Please ensure the questions highlighted on the image below are completed when admitting each patient.
