‘Patient Friendly Discharge Summary’ renamed ‘Patient Discharge Instructions’

To help simplify terminology and reduce confusion the Communicating for Safety Committee has recently endorsed the renaming of the ‘Patient Friendly Discharge Summary’ to ‘Patient Discharge Instructions’

The Patient Discharge Instructions are important in supporting tailored communication that ‘considers the health literacy needs of patients, families and carers’ and communicates inpatient and follow up care in a relevant, comprehensive way that ‘supports the persons understanding of information provided’ (ACSQHC, 2019).

To access the Patient Discharge Instructions go to:

  1. Nurse View in your Table of Contents
  2. Click on the Discharge tab
  3. Select Patient Discharge Instructions

The Patient Discharge Instructions will open up in Documentation, this can now be edited to provide the patient with important discharge information.

When completed Sign and Submit the form.

The Sign/Submit Note window will open.  Press the Sign & Print button.
