As part of preparations for the EMR Phase 2.1 Go-Live in July 2023, a short survey has been created to help understand the level of awareness across the organisation for this important project. 

The survey is just eight questions and will be submitted anonymously. 

The responses to the survey will help assist the Digital Health team with preparations and communications activities for the EMR Phase 2.1 Go-Live. 

The survey will run for two weeks and will close on 12th May 2023
